What Is a Home Care Package?

A Home Care Package is a coordinated package of support services, funded by the Australian Government, and tailored to your individual needs.
It allows you to choose the type of care and services that best suit your lifestyle — with Prestige Living Healthcare as your trusted provider.

There are four levels of Home Care Packages based on the amount of support you need — from basic assistance to high-level care.

Level 1 – Basic Support Needs

Best for: Older adults who need a little help to stay independent at home.

Services may include:

  • Light housework (vacuuming, laundry, dusting)
  • Meal preparation assistance
  • Occasional personal care (e.g. help with showering)
  • Social support and companionship
  • Transport to appointments

 

Ideal if you’re still mostly independent, but could use a helping hand.

Level 2 – Low Care Needs

Best for: Seniors who need regular assistance to manage day-to-day tasks.

Services may include:

  • Help with showering, grooming, or dressing
  • Meal planning and preparation
  • Medication reminders
  • Domestic support
  • Regular check-ins from care staff
  • Transport and escorted outings
  • Allied health (e.g., physiotherapy)

 

Perfect if you or your loved one needs more consistent support, but still wants to stay active and at home.

Level 3 – Intermediate Care Needs

Best for: People with more complex or frequent care needs.

Services may include:

  • Daily personal care (toileting, showering, continence support)
  • Nursing services and health monitoring
  • Dementia and memory support
  • Help with mobility and falls prevention
  • Managing chronic conditions (e.g. diabetes)
  • Coordination with doctors or specialists
  • Assistance with mobility equipment and home modifications

 

This level ensures safety, dignity, and stability for those who need more hands-on care.

Level 4 – High-Level Care Needs

Best for: Individuals with significant or complex health needs, including advanced age, multiple conditions, or palliative care.

Services may include:

  • High-frequency nursing and personal care
  • Medication and pain management
  • Palliative care support
  • Ongoing dementia care
  • 24/7 support planning
  • Coordination with specialists and allied health professionals
  • Intensive in-home assistance

 

This is the highest level of support available under the Home Care Package program — ideal for those who wish to remain at home with full support.

Upgrading Between Levels

Your care needs may change over time — and that’s okay.
We can help you:

  • Reassess your care needs
  • Apply for a higher-level package
  • Ensure continuity of care while waiting for approval

You’ll never be locked into one level — we’ll evolve with you.

Am I Eligible for a Home Care Package?

If you or a loved one is starting to find daily tasks more difficult — whether it’s getting dressed, preparing meals, or keeping the home clean — you may be eligible for government-funded support through a Home Care Package (HCP).

👵 Who Is Eligible?

You may qualify for a Home Care Package if you:

  • Are 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)
  • Are living at home and need assistance with daily activities
  • Are facing health challenges, mobility issues, or age-related conditions
  • Require ongoing care to remain safely and comfortably at home
  • Are already receiving care and feel your needs are increasing

 

You don’t need to be receiving a pension to qualify.
Packages are available to both full-pensioners and self-funded retirees.

How Is Eligibility Assessed?

To receive a Home Care Package, you’ll need to complete an ACAT (Aged Care Assessment Team) assessment, which is a free, in-person review of your care needs. It helps determine:

  • Your care level (from 1 to 4)
  • Whether you’re eligible for a package
  • What type of services will benefit you most

 

ACAT assessments are usually carried out in your home, by a healthcare professional like a nurse or social worker. You’re welcome to have a family member or support person with you.

What Will They Ask?

During your ACAT assessment, they’ll ask about:

  • Your health, medical conditions, and mobility
  • Whether you have any memory or cognitive challenges
  • How you manage daily activities like cooking, bathing, cleaning
  • Your living arrangements and social support
  • Any current care or services you already receive.

 

It’s not a test — just a caring conversation to understand what support would help you live well at home.

  • Offers flexibility as your circumstances change
  • Maximises your care hours by minimising unnecessary costs

 

We’ll also assign a dedicated care coordinator to support you on an ongoing basis.
